چکیده

Fractures of the proximal femur are a big challenge in traumatology. Rapid strides in implant and instrumentations in quest of ideal fixation of pertrochanteric and subtrochanteric femoral fractures have made various options available like Fixed-angled nail plate, Screw plates, Angled blade plate, Dynamic Hip Screw etc. The present study aims to study the role of standard PFN as a minimal invasive device in the management of these fractures. A total of 30 cases of closed fractures (18 pertrochanteric and 12 subtrochanteric) of age 18 or above of either sex were evaluated from 2008 to 2009. 72.2% of pertrotrochanteric and 50% of subtrochanteric fractures were of unstable type. Full weight bearing in 73.4% of the cases was possible at 8 weeks postoperatively. The fracture union in 90% of cases was seen in 12 weeks. 96.7% of cases achieved preinjury walking ability at the end of 24 weeks of follow up. The complications encountered were delayed union (2 cases), varus reduction (3 cases), difficult placement of neck screw/hip pin (2cases) and intra articular migration of neck screw was encountered in 1 case. The current study shows that PFN is an evolving approach to treat intertrochanteric and high subtrochanteric femoral fractures in a minimally invasive way and indicates PFN to be a safe and successful method. Varus collapse, difficulty in placement of neck/hip pin screws and intra articular migration of neck /hip pin screw might be the complications in very few cases which can be minimized by minimizing the technical errors while screw insertion.

The proximal femoral nail antirotation (PFNA) in the treatment of proximal femoral fractures.

Proximal femoral fractures, especially in elderly persons with osteoporosis, present a challenge for the traumatologist. While the dynamic hip screw (DHS) became the implant of choice for the treatment of stable fractures, the ideal implant for the treatment of unstable fractures remains an issue. In our experience, Proximal Femoral Nail Antirotation (PFNA) is an excellent device for osteosynth...

Failure of fixation of trochanteric femur fractures: Clinical recommendations for avoiding Z-effect and reverse Z-effect type complications

BACKGROUND Z-effect and reverse Z-effect are complications that arise from the surgical treatment of pertrochanteric fractures of the femur with proximal femoral nails (PFN) comprising two interlocking head screws. Such complications are induced by the migration of screws in opposite directions, which may lead to failure of the osteosynthesis. FINDINGS The paper describes three cases of pertr...

Management of Subtrochanteric Fracture Femur by Proximal Femoral Nail

Background: Subtrochanteric fractures of the femur remain some of the most challenging fractures faced by orthopaedic surgeons. Most of the fractures in the elderly results from trivial fall from standing or while walking, whereas in the younger age group it is mainly due to road traffic accidents. Closed management of these subtrochanteric fractures poses difficulties in obtaining and maintain...

Elastic Stable Intramedullary Nailing of Femoral Shaft Fracture-Experience in 48 Children

Background: Femoral shaft fractures are an incapacitating pediatric injury accounting for 1.6% of all pediatric bonyinjuries. Management of these fractures is largely directed by age, fracture pattern, associated injuries, built of the childand socioeconomic status of the family. We retrospectively evaluated the use of elastic stable intramedullary nail (ESIN)in surgical management of femoral s...

Concomitant ipsilateral femoral neck and shaft fractures--analysis of cases.

Concomitant ipsilateral shaft and femoral neck or pertrochanteric fractures are uncommon. They result from high-energy traumas and the mechanism of injury is usually complex. The proximal fracture is often not displaced and may be hardly visible on x-rays. Overlooking the proximal fracture may result in therapeutic problems. The best method of treatment is by closed stabilization of both fractu...
